Clinical Pharmacists

Clinical pharmacists work in primary in a patient facing role. They work with patients at GP practices to assess and treat patients using their expert knowledge of medicine. Clinical Pharmacists are prescribers, or are working towards an independent prescribing qualification. Typically Clinical Pharmacists take responsibility for patients with chronic diseases and undertake clinical medication reviews (CMRs) to proactively manage people with complex polypharmacy. Usullay this involves working with the  elderly, people in care homes and those with multiple comorbidities.
